Can I tour the facilities?
Absolutely! We would love to show you around. Please call
608-269-0386 to schedule a tour at either our Sparta or Bangor location.
Do you accept Medicaid?
We do not accept Medicaid. We accept private pay and Member Center Organizations. Please call us if you have questions.
Are your facilities fully wheel-chair accessible?
Our facilities are all ground level with full wheelchair accessibility. There are no stairs at any of our facilities.
Do residents have a private room?
All residents do have a private room with the exception of one double room located in our Bangor CBRF, perfect for a couple, siblings or friends.

What is the difference between an Adult Family Home and a Community Based Residential Facility?
Adult Family Homes (ADF) and Community-Based Residential Facilities (CBRF) are types of assisted living. Both are places where adults (ages 18+) who are not related to the operator reside and receive care, treatment or services that are above the level of room and board.
In a CBRF, up to three hours per week of nursing care may be provided to each resident. In an AFH, residents may receive up to seven hours per week of nursing care.
CBRFs range in size from 5 to 257 beds, while AFHs are limited to 4 beds.
